82-year-old man falls and hits head at casino
Heard on:
San Diego County CA ALS PAL (Hospital)
03:35
00:00
Palomar, medic 100 and fifty-one.
00:03
One hundred and fifty-one, this is Palomar.
00:04
It is 1318 .
00:07
Do you advise the R stroke five past?
00:11
Okay, Palomar, medic 100 and fifty-one.
00:13
That is pertinent information.
00:14
I am going to run this story by you real quick.
00:17
Kind of either major trauma or a stroke.
00:20
So just let me know if you want to divert to another hospital.
00:24
This is going to be an 82, 82 male.
00:26
Sounds like he was seated at a slot machine at the Pala Casino.
00:30
I'll give you a run number, everything shortly.
00:32
But he was seated at the slot machine when he fell off the slot machine, forward, hit his head.
00:37
He does have a hematoma about 1 to 2 inches above his right eyebrow.
00:41
Per witnesses, they were not really great.
00:43
It sounds like possibly might have had 3 min of seizure activity, but they weren't really describing like generalized tonic-clonic seizure activity.
00:51
They're saying he's really like tensed up.
00:55
When we got there, patient is non-verbal for us.
01:01
He's moving all extremities times four spontaneously.
01:06
As far as his GCS, his eyes are in four.
01:11
Verbal is going to be zero, and motor is going to be five.
01:24
As of right now, it appears that his pupils are different sizes.
01:28
His right pupil is about four.
01:31
Pupil's about 2 mm .
01:36
As far as vitals, everything's stable within normal limits except for a blood pressure of 206 over 91 .
01:44
So based on the signs of being very hypertensive and the unequal pupils and being aphasic, obviously with outward signs of trauma and the mechanism of injury to the head, we were going to have to send this to your facility as a major trauma.
01:58
But obviously with the chance this could potentially be stroke, do you want us to divert to a trauma center that is currently open for stroke? We have about a thirty-three zero minute ETA to your facility.
02:11
Stand by just 1 s .
02:15
I copy that.
02:16
Remainder of the FASTED is negative, except for the AFIB.
02:21
Can you repeat the blood pressure for me?
02:24
Most recent blood pressure was 206-206 over 91.
02:29
On scene our blood pressure was 220-220 over 112.
02:35
Okay, copy that.
02:36
Yeah, we'll go ahead and make this a major trauma activation.
02:39
If you could give me a 10 min heads up or recontact with any changes, I would appreciate that.
02:47
That's affirmative. I copy that information.
02:49
Additionally, just also to let you guys know, all of his phone information is in Mandarin, so I'm not sure if maybe he's just not talking to me because he is just not English speaking, or if it's trauma related.
03:02
But obviously, with him being very hypertensive and the period of unconscious, possibly seizure activity, well, I'm comfortable running this as a major trauma as well.
03:13
Okay, copy that.
03:14
Oh, and also, do you have a run number yet, or do you need to get that to me on arrival?
03:19
Yes, affirmative.
03:20
Run number 2026-114533.
03:28
Copy that, 4533.
03:29
We'll see you inside in about 30 minutes.
03:32
Recontact with any changes, and then a 10-minute heads up.
