This is JCA 3815 coming in non-emergent with an 84-year-old male coming from home.

Chief complaint today is going to be gastric reflux and patient is complaining that he cannot move his whole body.

Patient does have a history of cerebral.

He is able to move around.

He just feels like he is more weak than normal.

His at-home nurse stated that he had a medication change recently, a couple weeks ago, and had an episode like this then.

Patient said he feels similar at that time as well.
