Two complaints is going to be able to fall with disorientation. Patient is currently conscious, alert-oriented. GCS was 15. She's napping at a moment, but she will arouse if we try to talk to her. Unwitness fall. Approximately this morning, patient believes she remembers falling. She doesn't know if she hit her head. Complaining of neck pain, though. We do have her coward at this time. Last time she was heard from was last night. When family came and found her today, they found her on the floor. Very disoriented, not oriented at all. I can go anything out of consciousness. Current vitals are a heart rate of 75 sinus on the monitor, 100% on 4 liters, nasal cannula. Blood pressure currently 90 over 40. That's after a 1 liter bag of fluid. When I got to her, original systolic was 78 with no radial pulses. Blood Sugar, 131, 131.
